The requirements for PS Form 3624 United States Postal Service nonprofit organizations are as follows:
1. The organization must be officially recognized by the IRS as a 501(c)(3) or 501(c)(4) organization.
2. The organization must have a valid Employer Identification Number (EIN).
3. A valid address and contact information (including email address) must be provided.
4. The organization must provide proof of its charitable mission, such as a letter from the IRS confirming its 501(c)(3) or 501(c)(4) status or a copy of its mission statement.
5. The organization must complete the USPS Charity Application form 3624.
6. Once approved, the charity will receive an authorization number which can be used to apply for certain USPS services and discounts.
